INSTRUCTIONS
- Please choose your preferred date below. Make sure to be present on the selected schedule. Otherwise, your reservation will be cancelled.
- Do not make multiple reservations. If two or more reservations are made in different schedule, POLO shall confirm the earliest available schedule and cancel other reservations.
IMPORTANT REMINDERS
- You must be 50 years old and above or will be turning 50 years old this year to avail the Vaccination Subsidy Scheme of the Department of Health.
- If your age is below 50 years old, you may also avail the flu vaccine. However, Shoebill Health Care Group will charge HKD200.
- Medical assessment will solely be done by Shoebill Health Care team on the day of your appointment to know if you are qualified to receive the vaccine.
- Please bring the following documents :
- Original Hong Kong Identity Card
- Vaccination record (if applicable).
