Application for Dual / Multiple Accreditation
A Foreign Placement Agency (FPA) may be accredited to four (4) licensed recruitment agencies while the Principal/Employer may be accredited up to a maximum of five (5) licensed recruitment agencies.

PART A. (ONE COPY ONLY of the following):
- Letter Request for verification of accreditation of documents.
- Information Sheet (form provided, please fill up completely).
- HK ID of the owner/s OR licensee/s and recent one 2x2 photograph
- License copy of Philippine Agency (PRA)
- Self-Declaration of No Criminal Record
- Photo of the Office Facilities & Boarding House
- Lease Agreement (Office & Boarding House)

- Recruitment Agreement with Philippine counterpart
- Special Power of Attorney (SPA-FRA)
- Special Power of Attorney (SPA-PRA)
- Joint Affidavit
- Agency Undertaking
- Sworn Statement
- Job Order
- Contingency Plan
- Business Registration
- License to Operate an Employment Agency
- Master Employment Contract (BLUE CONTRACT - FOUR SETS with Agency chop)
- Deployment Report certified by POEA ICT Branch (covering two (2) years prior to application for DUAL)
- FRA’s Owner/Representation “Appointment of Filipino Welfare Officer” (attach Resume, training certificates, if there’s any)
